Job Title:Care Coordinator

Working Hours: Monday- Friday9am- 5:30 pm (40 hours per week)

Accountability: Registered Manager

Location: WelTech BusinessCentre, Welwyn Garden City AL7 2AA

Liaison: Communicate withclients, clients' representatives, care workers, Care Managers, andother professionals (eg, Social Workers).

MainResponsibilities:

  • Ensure care worker visits to clients reflect thecontinuity and flexibility Hartwig Care promises.
  • Ensure visits occur on time and with the appropriate careworkers tailored to clients' needs.
  • Update allcare workers with necessary information regarding client visits andcompany policies.
  • Receive information/reportsfrom care workers verbally and in writing within settimelines.
  • Coordinate care worker reviewbookings and team meetings, ensuring they happen as
  • Cross-reference actual bookings against planned bookingsfor accuracy.
  • Aid in improving servicemonitoring and client safety.
  • Assist CareManagers in communicating with care workers regarding daily visitsand meetings.
  • Answer phone calls and textmessages, take messages, and redirect calls within the telephonesystem.
  • Care worker issues regarding clients -Client schedule.
  • Care worker's own issues -Care Worker schedule.
  • Any complaints,concerns, or compliments - handover notes/email/quicknotes
  • Issues needing follow-up - forward torelevant people via email, handover notes, and quicknotes.
  • Confirm all rotas have been receivedand hours accepted.
  • Send the weekly rota toall clients by post.
  • Handle day-to-dayallocations of visits.
  • Ensure clients areinformed of changes to their weekly rota.
  • Ensure HCR holds carers' right preferredavailability.
  • Assist in identifying when newcare workers are needed.

Ideal candidate qualitiesinclude:

  • Validdriver's licence business insurance preferred.
  • It is a fast- paced environment, the successful CareCoordinator will be interpersonally confident and a pro-active teamplayer.
  • Discretion and the ability to handleconfidential matters and sensitively.
  • Have theability to thrive under pressure and good organisational andmultitasking skills.
  • Showcase a high level ofattention to detail and be able to prioritise workload.
  • Have excellent communication skills, previous experienceand always professional and polite.
  • Competencein Microsoft Office products: Excel, Word, MicrosoftOutlook
  • Proactive and ability to useinitiatives during complex situations.
